Information about typical violations that resulted in the death (injury) of employees working in organizations of the Minsk region, and measures

for their prevention

04.07.2019 At the construction site  Construction of a plant for

the production of sheet metal from tinplate in the city of Miory, Vitebsk region. Sheet rolling shop (LPC) workers

UP  SU No. 20 of JSC Minskpromstroy (Dzerzhinsk district) , on the instructions and under the guidance of a master, performed work on reinforcing the walls of the cable well KKm-11. Since the dimensions of the pit did not allow the stack of the reinforcing rod to be lowered into the pit with the help of a crane, the electric and gas welder and the carpenter moved and lowered it manually.

Due to the fact that the upper bar of the protective fence installed

on the edge of the pit prevented the rod from being fed down, the electric and gas welder removed it. During the next transfer of reinforcing rods to an employee who was in the pit, the electric and gas welder could not resist and fell

into it to a depth of 2.3 m, having received a serious industrial injury. The causes of this accident:

violation by the master of labor protection duties, expressed

in the absence of constant control on his part over the implementation of measures provided for in the work permit and the work production project to ensure the safe production of work

with increased danger, which led to the removal of the upper horizontal bar of the protective fence of the pit;

violation by the victim of the requirements of local regulations on labor protection, expressed in the removal of the upper horizontal bar of the protective fence of the pit.

In order to prevent such accidents, it is necessary:

update the instructions on labor protection for professions and (or) certain types of work (services) in terms of introducing requirements to

prohibit employees from removing, changing or rearranging safety equipment and other devices designed to protect

themselves or other employees;

to ensure that work with increased risk is carried out under the supervision of the line managers of the work under the direct supervision of the line managers of the work, the implementation of constant monitoring of compliance by employees with the requirements of the instructions on labor protection, work production projects

and technological maps;

to bring employees to disciplinary responsibility

for untimely notification to the employer about any situation that threatens the life or health of employees and others,

including for violations of labor protection requirements by other employees within the framework of concern for the safety of others in the process of performing work or during their stay on the territory of the organization.

24.07.2019 in LLC Borisovskaya Concrete Company, the driver of the mobile aggregate complex for the preparation of mortar-concrete mixtures and the driver of the loader carried out preventive work on the concrete mixing unit of the mobile aggregate complex MAC-Concrete (cleaning the working surfaces of the concrete mixer from stuck concrete and fragments of mixtures), as well as replacing the mixer scrapers. The workers opened the valves of the mixer (hatches), after which the loader driver descended through the unloading hole of the concrete mixer and began cleaning its working surfaces

and scrapers with a puncher.At this moment, there was a power outage. When the power was cut off, the valve of the mixer gate began to close, which caused the chest of the loader driver to be squeezed.The victim died from his injuries on 23.08.2019, he was in the intensive care unit of the Minsk Regional Clinical Hospital.

The causes of this accident:

violation of safety requirements during maintenance and repair of the concrete mixer of the mobile aggregate complex MAC-Concrete , expressed in the performance of work on the concrete mixer without disconnecting the supply of electrical power to the solenoid that drives

the pneumatic distributor and, accordingly, the pneumatic cylinder, as well as in the failure to block the supply of excess air pressure in the pneumatic cylinder that drives the gate of the concrete mixer bowl in an open hole in the bottom of the bowl;

involvement of the victim to work on the maintenance and repair of a concrete mixer that does not have the appropriate qualification in the profession of a repairman;

admission of the victim to work on the maintenance and repair of concrete mixers without training, internship, briefing and verification of knowledge on labor protection issues.

The director of LLC Borisovskaya Concrete Company was identified as the person who committed violations of labor protection legislation, technical regulatory legal acts.

In order to prevent such accidents, it is necessary:

to ensure the development of local normative legal acts (instructions on labor protection, technological instructions, and others) containing requirements for labor protection during operation, maintenance and repair of equipment, set forth in the operational documents of the manufacturing organizations of the equipment operated in the organization;

to allow employees who have the appropriate qualifications in the profession, who have passed

training, internship, instruction and knowledge testing on labor protection issues in accordance with the established procedure, to service and repair equipment.

04.01.2020 The head of the forest point of the GLHU Logoysky Forestry

by phone instructed the master at the cutting areas together with the forest fellers to continue the work on cutting down wood and shrub vegetation on the lands of LLC KPI SpeshLprojects to install an external perimeter fence. After inspecting the site, there were no dangerous trees and, waiting for daylight, the forest fellers began to work on cutting down trees and pruning branches.In the process of work, one of the fellers of the forest cut down the aspen moved away from the direction of the felling, and the aspen when falling with its crown caught two trees standing in front of it, which fell to the ground together with the aspen (tearing their root system out of the ground). A piece of fallen and broken wood felled the woodcutter at his feet, seriously injuring his chest.

The cause of this accident was a violation by the victim of the requirements of local regulations on labor protection:

the workplace was not prepared (branches (felling residues) were not removed)at a distance of 70 cm), and a waste path of at least

4 m and at an angle of45°in the direction opposite to the fall of the tree;

the plane saw is not made perpendicular to the axis of the tree (the slope to the center of the stump), the half-saw is not left 3-9 cm wide;

the felling of the tree is made on the wall of the forest, and not in the gap between neighboring trees.

In order to prevent such accidents, it is necessary to ensure that high-quality explanatory work is carried out among employees about the need to take care of personal safety and personal health, as well as about the safety of others in the process of performing work or during their stay on the territory of the organization.
